A recent scientific presentation by Andrea Romanos-Nanclares at the Olive Oil and Health in Brazil Symposium 2025 explored one of the most discussed topics in modern nutritional science:

Can olive oil consumption influence cancer risk and long-term health outcomes?

The presentation brings together evidence from large epidemiological studies, randomized clinical trials and mechanistic research surrounding extra virgin olive oil (EVOO), Mediterranean dietary patterns and olive-derived polyphenols.

Olive oil is increasingly being studied not merely as a culinary fat, but as a biologically active component within a broader Mediterranean lifestyle associated with healthy aging, metabolic resilience and reduced chronic disease risk.

Olive Oil and Cancer: What the Evidence Suggests

One of the central themes of the presentation is the growing body of evidence linking higher olive oil consumption with lower cancer risk.

A major systematic review and meta-analysis presented in the lecture reported that individuals with the highest olive oil intake showed a:

  • 31% lower likelihood of developing cancer overall

compared with those consuming the least olive oil.

The strongest associations were observed for:

  • breast cancer
  • gastrointestinal cancers
  • upper aerodigestive cancers
  • urinary tract cancers

Importantly, the presentation repeatedly emphasizes that these findings are primarily observational and therefore cannot prove direct causation. However, the consistency of the associations across multiple populations continues to attract scientific attention.

Breast Cancer and Olive Oil

A substantial portion of the presentation focused on breast cancer research.

Researchers observed that Mediterranean populations — where olive oil is a central dietary fat — generally show lower breast cancer incidence, including lower rates of more aggressive tumor types.

One particularly important study highlighted was the Italian Moli-sani Study, involving more than 11,000 women followed over 13 years.

The study found that:

  • women consuming more than 3 tablespoons of olive oil daily showed a 29% lower breast cancer risk compared with women consuming 2 tablespoons or less.
  • each additional tablespoon of olive oil was associated with lower risk of certain aggressive breast cancer subtypes, including ER-negative and HER2-negative cancers.

At the same time, the presentation also discusses scientific nuance.

Large U.S. Harvard cohort studies involving over 160,000 women did not find a statistically significant association between olive oil intake and overall breast cancer risk.

Why the difference?

According to the presentation, several important factors may explain this:

  • overall olive oil intake in U.S. populations was relatively low
  • studies often could not distinguish between extra virgin olive oil and refined olive oil
  • intake of olive polyphenols was likely much lower in non-Mediterranean populations

The PREDIMED Study: A Landmark Trial

One of the strongest findings presented came from the famous Spanish PREDIMED trial.

In this randomized clinical trial:

  • women assigned to a Mediterranean diet enriched with polyphenol-rich extra virgin olive oil showed a 62% lower breast cancer risk compared with control participants.

Unlike observational studies, participants in PREDIMED received controlled amounts of high-quality EVOO with substantially higher polyphenol intake.

This may partly explain why the protective associations appeared stronger than in some observational studies using mixed olive oil categories.

Olive Polyphenols and Cancer Biology

The presentation also reviewed experimental research on olive-derived polyphenols including:

Laboratory studies suggest these compounds may interact with multiple biological pathways related to cancer development, including:

  • oxidative stress
  • inflammation
  • cell proliferation
  • angiogenesis
  • apoptosis
  • metastasis pathways

The presentation summarized evidence suggesting olive polyphenols may:

  • support antioxidant defense systems
  • influence inflammatory signaling
  • interact with tumor-related cellular pathways

However, OHI stresses that most of these mechanistic findings come from laboratory and preclinical studies, not human therapeutic trials.

This means olive polyphenols should not be presented as cancer treatments.

Instead, the evidence suggests they may contribute to a healthier biological environment within the context of long-term dietary patterns.

Beyond Cancer: Broader Health Effects

The presentation also highlighted evidence from large Harvard cohort studies showing that higher olive oil intake was associated with:

  • lower total mortality
  • lower cardiovascular mortality
  • lower neurodegenerative disease mortality
  • lower cancer-related mortality
  • lower respiratory disease mortality

One major analysis involving over 92,000 participants reported that individuals consuming more than 7 grams of olive oil daily had significantly lower dementia-related mortality.

Other findings suggested that replacing butter, margarine or dairy fats with olive oil was associated with lower mortality risk overall.

The presentation also discussed newer evidence suggesting olive oil intake may be associated with:

  • healthier body weight regulation over time
  • improved metabolic markers
  • better lipid profiles
  • healthier inflammatory balance

Why Extra Virgin Olive Oil Matters

A recurring theme throughout the presentation is the distinction between:

  • refined olive oils
  • and high-polyphenol extra virgin olive oils

Extra virgin olive oil naturally contains:

  • monounsaturated fatty acids (MUFA)
  • polyphenols
  • tocopherols
  • squalene
  • minor lipid bioactives

Researchers increasingly believe these compounds may work together synergistically.
