A newly published study in BMC Medicine (2025) provides further evidence that the Mediterranean diet may play an important role in supporting long-term cognitive health through its interaction with the gut microbiota.

The research, conducted within the large Spanish PREDIMED-Plus cohort, explored how adherence to the Mediterranean diet influences gut microbial composition and how these microbial patterns are associated with cognitive decline over time in older adults with overweight, obesity and metabolic syndrome.

For the Olive Health Institute, this study reinforces an increasingly important scientific concept:
the relationship between nutrition, the gut microbiome and brain health is not isolated, but part of a deeply interconnected biological network often referred to as the microbiota–gut–brain axis.

The Mediterranean Diet as a Neuroprotective Lifestyle Pattern

The Mediterranean diet has long been associated with healthy aging, cardiovascular support and reduced risk of chronic disease. This dietary pattern is characterised by:

  • High intake of vegetables, fruits, legumes, nuts and whole grains
  • Frequent use of extra virgin olive oil as the primary fat source
  • Moderate fish consumption
  • Limited intake of processed foods, red meat and saturated fats

Importantly, the study highlights that these dietary habits may influence cognitive aging not only directly, but also indirectly through modulation of the gut microbiota.

The researchers followed 746 adults over a 6-year period and repeatedly assessed cognitive performance using extensive neuropsychological testing. At the same time, gut microbiota composition was analysed using advanced 16S rRNA sequencing technologies.

Gut Microbiota: A Missing Link Between Diet and Brain Function

One of the most important findings of the study is that higher adherence to the Mediterranean diet was associated with:

  • Greater microbial diversity
  • More favourable microbial composition
  • Higher abundance of beneficial short-chain fatty acid (SCFA)-producing bacteria
  • Lower abundance of pro-inflammatory bacterial taxa

Microbial diversity is increasingly viewed as a hallmark of a resilient and metabolically healthy gut ecosystem. In this study, participants with stronger Mediterranean diet adherence demonstrated significantly higher gut microbial richness and diversity scores.

Among the beneficial bacterial genera associated with Mediterranean diet adherence were:

  • Barnesiella
  • Butyricicoccus
  • Agathobacter
  • Bacteroides

These bacteria are known for producing short-chain fatty acids such as butyrate, compounds increasingly studied for their role in:

  • Gut barrier integrity
  • Immune regulation
  • Reduction of low-grade inflammation
  • Communication with the central nervous system

Conversely, bacterial genera associated with inflammatory and metabolically adverse conditions — including Eggerthella and Enterococcus — were inversely associated with Mediterranean diet adherence.

Olive Oil and Polyphenols as Key Drivers

One particularly relevant observation for the Olive Health Institute is that olive oil, vegetables and wine emerged as major contributors influencing microbial composition within the Mediterranean dietary pattern.

These foods share an important characteristic: they are rich in polyphenols.

Polyphenols are bioactive plant compounds increasingly recognised for their antioxidant and prebiotic properties. Because many dietary polyphenols are only partially absorbed in the upper digestive tract, significant amounts reach the colon where they interact directly with gut microbiota.

The study discusses how gut microbes metabolise these compounds into smaller phenolic metabolites with potential biological activity linked to:

  • Oxidative balance
  • Immune modulation
  • Gut barrier maintenance
  • Neuroprotection

This aligns with a growing body of evidence suggesting that high-polyphenol extra virgin olive oil may influence health far beyond its fatty acid composition alone.

For OHI, this reinforces the importance of considering EVOO not merely as a culinary fat, but as a complex biological matrix rich in interacting bioactive compounds.

Cognitive Decline and the Gut–Brain Axis

Perhaps the most significant outcome of the study is the observed association between Mediterranean diet adherence and slower cognitive decline over six years.

Both:

  • self-reported Mediterranean diet adherence (MEDAS score), and
  • a newly developed microbiome-derived Mediterranean diet signature (MedDiet-GMS)

were independently associated with more favourable cognitive trajectories over time.

The strongest associations were observed for:

  • Global cognitive function
  • Executive function
  • General cognitive performance

The microbiome-derived score is particularly interesting because it suggests that gut microbial composition itself may become a measurable biomarker reflecting long-term dietary quality and potentially cognitive resilience.

The researchers also identified Eggerthella as a microbial genus associated with greater executive function decline. This bacterium has previously been linked to inflammatory states and production of potentially neuroactive metabolites.

Why This Matters

The study population consisted of older adults with obesity and metabolic syndrome — a group known to be at increased risk for both cognitive decline and microbiota dysregulation.

This makes the findings particularly relevant in the modern context where metabolic dysfunction, ultra-processed food consumption and chronic low-grade inflammation are increasingly prevalent.

The study supports a broader scientific narrative:

Modern chronic diseases may not be isolated disorders, but interconnected disturbances involving metabolism, inflammation, microbiota and lifestyle.

Within this framework, the Mediterranean diet appears to act not only as a nutritional pattern, but as an ecological regulator of the human microbiome.

For the Olive Health Institute, this research further strengthens the emerging concept that extra virgin olive oil — particularly polyphenol-rich EVOO — should be viewed within the larger context of systems biology and One Health.

The future of nutritional science increasingly lies in understanding:

  • inflammation and oxidative stress pathways
  • personalized nutrition
  • healthy cognitive aging

This study does not claim that olive oil or the Mediterranean diet "prevents" dementia. However, it contributes to the growing scientific evidence suggesting that dietary patterns rich in plant foods and olive-derived polyphenols may support a more favourable biological environment associated with healthy aging and cognitive resilience.

As science continues to explore the microbiota–gut–brain axis, extra virgin olive oil remains one of the most biologically complex and scientifically intriguing components of the Mediterranean lifestyle.
